- 1 photo (size 3 X 4 cm) taken within 3 months
Name of applicant and affiliation should be written on reverse side.
- Receipt of bank transfer(to be pasted in the application form)

Cancellation:
If you wish to cancel the application, please notify JB-POT Secretariat by fax, letter or E-mail.
Please note that no refund of examination fee already paid will be made in case of cancellation.

10,000 yen (Qualifying certificate and pin badge will be given to successful candidates)
